Palos Health Patient Portal Terms of Use

Welcome to the Palos Health Patient Portal web site (also referred to as the "Patient Portal"). The Patient Portal is made available to our patients subject to each users agreement and compliance with the terms and conditions set forth below. Please read these Terms of Use, the Legal Notice available for review at http://www.paloshealth.org/disclaimer/ and the Privacy Policy available for review at http://www.paloshealth.org/about-us/privacy-practices carefully. By using this Patient Portal, you acknowledge that you have read, understand and agree to be legally bound by all of the terms and conditions set forth in these Terms of Use, the Legal Notice and the Privacy Policy. Palos Health reserves the right to amend or modify the Terms of Use, Legal Notice and Privacy Policy from time to time by posting the amended or modified terms on this Patient Portal. You should check the Terms of Use, the Legal Notice and Privacy Policy frequently for any such changes, but in any case, your continued use of the Patient Portal constitutes your agreement and acceptance of any modification of the Terms of Use, Legal Notice and Privacy Policy. Palos Health is pleased to offer you access to the Patient Portal and the features you can use to access information regarding portions of your medical records and related information and resources. The Patient Portal is intended to enable you to:

  • View select portions of your medical record
  • View certain of your lab results
  • View allergies
  • View visit records
  • View and update your demographic information
  • View medications as of your last inpatient visit
  • View upcoming appointments

A. Privacy Policy
Palos Health will securely maintain your confidential medical information and will not disclose any such information except in accordance with our Privacy Policy. Please review our Privacy Policy for a more thorough description of how we gather, use and protect your confidential medical information.
B. Portal Access
Children under the age of 12 may not use the Patient Portal and parents or legal guardians may not agree to these Terms on their behalf. If we become aware that a child under the age of 12 has provided or attempted to provide us with personal information, we will use our best efforts to remove the information permanently from our files. Portal access for patients between ages 12-17 may require both parental/legal guardian and patient consent. 
C. Account Activation
When you initially enroll in the Patient Portal, you will be provided with a unique onetime username and password by email to activate your account. The username and password you receive will give you the opportunity to create a Logon ID ("Logon Id") and password that only you shall know. Once the username and password we provide you have been replaced with your personal Logon Id and password, the unique onetime username and password will no longer be valid. You will be asked to provide your e-mail address when registering your account. It is your responsibility to update this e-mail address as appropriate. If you need to change your email address, you must come to the information desk at the main entrance of the hospital. It is extremely important that you keep your Logon Id and password completely confidential. Anyone with access to your Logon Id and password will be able to access your confidential medical information. It is your responsibility to prevent disclosure of your login and password, and to change your password if you feel that your security has been compromised. You may change your password at any time. When you access the Patient Portal, you will find a Frequently Asked Questions (FAQ) link to assist you in navigating its features and offerings. By accepting these Terms of Use, you are acknowledging and agreeing that you are solely responsible for any sharing of Patient Portal content or information that you intentionally or unintentionally communicate to others. You agree to immediately notify Palos Health in the event (1) your account information changes or (2) you learn of or have reason to suspect any unauthorized use of your account or any other breach of security. If you feel your security has been compromised, you may change your password at any time.
D. Emergency or Urgent Matters
The Patient Portal should never be used for emergency or urgent matters. If you believe you need to speak to your physician or other health care provider on an urgent basis, please do not use the Patient Portal. For all medical emergencies and other urgent matters, please contact your physician’s office by phone, go to an emergency room, or dial 911. The Patient Portal is not a substitute for timely contact with your physician or health care provider.

E. Email Notifications
The Patient Portal will notify you when new information is available in your Patient Portal account. Any such notifications will be delivered to the email address specified by you at the time of account registration. You agree that we may provide to you notices and other information concerning the Patient Portal electronically, including notice to any email address that you may provide, in accordance with the Privacy Policy.
F. Medical Disclaimers
The Patient Portal provides you with access to select, limited health information from your patient medical record. However, the Patient Portal does not reflect the complete contents of the medical record. The Patient Portal is designed for your convenience and enhances your access to certain parts of your medical record and other health care information. A complete copy of your medical record can be requested from your health care provider. Palos Health assumes no responsibility for how you use the information you obtain from the Patient Portal. The information provided on the Patient Portal is not a substitute for the advice of your personal physician or other qualified health-care professional. Always seek the advice of your physician or other qualified health-care professional with any questions you may have regarding medical symptoms or a medical condition. Never disregard professional medical advice or delay in seeking it because of something you have read on or accessed through the Patient Portal. The Patient Portal is offered as a courtesy for informational and educational purposes only and Palos Health does not guarantee that you will be able to access the Patient Portal, or that the information you access is complete or accurate. It is possible that the Patient Portal may include typographical, truncation, transmission or other errors or inaccuracies. Information available on the Patient Portal may be updated without notice.
G. Termination of Access
Palos Health may restrict or terminate your access to the Patient Portal at any time, with or without notice to you, including if you violate these Terms of Use. It is necessary to restrict access to the Patient Portal on occasions when system maintenance is being performed. Reasonable effort will be made by Palos Health to minimize these system downtimes. The Patient Portal is a secure internet-based environment through which you can access your health information. While the Patient Portal uses encryption software, if you use the Patient Portal and transfer information over media that are beyond the control of Palos Health, its contractors and subcontractors it may not remain secure. You are responsible for ensuring that the computer systems you use to access the Patient Portal have appropriate security controls enabled and you expressly assume the risk of any unauthorized disclosure or intentional intrusion, or of any delay, failure, interruption or corruption of data or other information transmitted in connection with the use of the Patient Portal. You are solely responsible for providing, maintaining and ensuring the compatibility of all hardware, software, electrical and other physical requirements necessary for your access to and use of the Patient Portal, or any part thereof. Once the information is received by Palos Health, your health information will be treated as confidential in the same manner as a medical record.
H. Links to Third-Party Websites
The Patient Portal may contain links to websites operated by third parties or materials supplied by third parties. Any such third party information is provided for your convenience only and you should not rely on the information contained in these materials or websites for purposes of treatment or diagnosis. Palos Health does not sponsor, operate, control or endorse any of these third parties nor the information, products or services provided by third parties. Palos Health does not make any guarantee, warranty or representation regarding the accuracy of the information contained on third party websites or provided by third parties. Palos Health has no control over the security or privacy practices of third party websites and use of these websites is at your own risk including, but not limited to, any risks associated with destructive viruses or computer code. Palos Health disclaims any responsibility for or liability related to such third party websites and materials.
I. Proprietary Rights
All content included on the Patient Portal, including, but not limited to, text, photographs, graphics, icon buttons, images, artwork, names, logos, trademarks, service marks and data ("Content") in any form, including compilations, are protected by U.S. and international copyright and trademark laws. The Content includes Content owned or controlled by Palos Health and/or by third parties and licensed to Palos Health and no right, title or interest is granted to you in or to any Content other than the right to use such Content in connection with your use of the Patient Portal. Reproduction of any Content, in whole or in part, by any means, is prohibited. The Patient Portal and the Content available through the Patient Portal is made available by Palos Health for patients only. All rights not expressly granted are reserved by Palos Health and its suppliers. The words "Palos Health" and its associated corporate logos and names are the trademarks and service marks of Palos Health. All other trademarks, service marks, and logos are the trademarks, service marks, or logos of their respective owners. Nothing contained in the Patient Portal should be construed as granting any license or right to use any trademark, service mark, or logo displayed on the Patient Portal without the express permission of Palos Health or the third party that may own the trademark, service mark, or logo.
J. Miscellaneous
You may discontinue your use of the Patient Portal at any time by contacting Portal Support at 708-923-4300 The Terms of Use, Legal Notice and the Privacy Policy, as may be amended as set forth above, constitute a binding agreement between you and Palos Health, its affiliates, physicians, employees, managers, officers, directors, agents and representatives. If you agree to the above Terms of Use, Legal Notice and Privacy Policy, please indicate your agreement by clicking the box labeled "Accept." If you do not agree to the Terms of Use, Privacy Policy and Patient Portal Access Agreement, please click "Decline" and you may not use the Patient Portal.
